Order Forms
How to get free rapid tests for your business
- First, assign a Screening Supervisor Designate who will be the contact person on tests.
- The Screening Supervisor Designate must watch this 6-minute training video on how to administer the tests.
- Complete the rapid test order form. If you have multiple businesses, fill out one form per business. If you have one business with multiple locations, please fill out one form covering all locations.
- The number of tests you receive will be automatically calculated based on the number of employees you provide in the order form. Orders are calculated to provide enough rapid tests to screen all employees twice a week for four weeks.
- Once your order is received, your pick-up time and location will be confirmed by the Toronto Region Board of Trade. Add rapidscreening@bot.com to your “Safe Senders” list on email to make sure your receive time-sensitive communications and don’t miss important updates.
You cannot order more tests unless you report the results — reporting the results is required. Once your results are submitted, you can reorder more tests through our rapid test refill form.
Businesses are also responsible for the proper disposal of tests. They cannot be put out with your usual waste, so please follow the Ontario government's guidelines on how to safely manage and dispose rapid antigen tests.
How to report rapid test results
To get free rapid tests, business must agree to report the results of those tests weekly. The Screening Supervisor Designate is responsible for reporting all results.
Every day, track the:
- Number of rapid antigen tests used
- Number of invalid rapid antigen test results
- Number of individuals who tested positive with a rapid antigen test
- Number of individuals who tested negative with a rapid antigen test
Next, report your results through our online results portal — these results are shared with Health Canada and the Ontario Ministry of Health.
Note: results must be reported before you can submit a refill order.
Remember, people who test positive for COVID-19 through rapid antigen test should confirm with a PCR test at any assessment centre, preferably within 24 hours of the rapid test. While waiting for the confirmatory test results, you must isolate at home, whether you have symptoms or not, for 10 days from the first day your symptoms started or the date of your positive rapid test if you didn’t have any symptoms.
Guidelines for picking up tests
Do not visit the pick-up location until you have received a confirmation email with your confirmed pick-up time. To ensure public health and safety, pick-ups are designated to 15-minute windows. Please arrive on time to pick up your order.
If you need to enter a City building to collect your kits, first self-screen for COVID-19 symptoms using the QR code posted on the door. Do not arrive at the pick-up facility of you are experiencing symptoms.
Wear a mask or face covering, and maintain 2 metres of physical distance at all times.
